Initial Consultation with Surgeon and Emotional Well-Being Practitioner (EWP)

The patient’s journey begins with a comprehensive first consultation involving both a surgeon and an EWP.

During this session, the surgeon assesses the medical aspects of the patient’s condition and discusses potential surgical options.

The EWP offers the emotional and psychological support and works with patients to plan the necessary strategies to manage anxiety and stress related to the upcoming procedure.

Second Consultation with Surgeon and EWP

A follow-up consultation allows the patient to clarify any doubts and further discuss the details of the proposed surgical plan.

This session also provides an opportunity for the EWP to reinforce coping strategies and ensure the patient’s emotional concerns are addressed, adjusting support mechanisms as needed.

Additional Consultation with EWP (if necessary)

If there are ongoing emotional or psychological concerns, a separate consultation with the EWP can be arranged.

This session focuses solely on providing targeted emotional support and strategies to help manage specific issues such as preoperative anxiety or fear.

Pre-Operative assessment and introduction

Prior to surgery, patients meet with the nursing team for a pre-operative briefing. This includes a review of the patient’s medical history, confirmation of surgical plans, and instructions about pre-surgery preparations.

Nurses also discuss post-operative care and recovery expectations.

Surgery

The patient undergoes the planned surgical procedure. Surgical teams are highly trained and utilise state-of-the-art technology and techniques to ensure optimal outcomes.

Post-Operative Follow-Up with Nurses (first week)

Within the first week after surgery, the patient has a post-operative appointment with the nursing team to assess initial recovery, manage any immediate post-surgical concerns such as pain or wound care, and evaluate the need for further medical support.

Post-Operative Follow-Up with Surgeon and EWP (6 weeks)

Six weeks post-surgery, the patient meets both the surgeon and the EWP.

The surgeon reviews the surgical outcomes and physical recovery, while the EWP addresses any ongoing emotional or psychological needs, helping the patient adjust to post-surgery life.

Subsequent Post-Operative Follow-Ups with Surgeon and EWP (3 month, 6 month, 12 month) if needed

Patients’ follow up is tailored according to their need and the type of surgery they had.  Follow-up appointments are usually booked at three, six, and twelve months post-surgery with the surgeon.

These appointments are crucial for monitoring long-term recovery and addressing any late-arising complications or concerns.

Discharge

Upon satisfactory recovery and resolution of all immediate surgical concerns, the patient is officially discharged.

Discharge planning includes a final review of the patient’s surgical outcomes, long-term care recommendations, and any necessary referrals for additional support.

Ongoing Access to Support

Throughout their journey, patients have continuous access to the surgical, nursing teams and Emotional Well-Being Practitioner.
